CLINIC MANAGER FLSA Status: Non-Exempt Exempt Approved Date: January 1 2019 PURPOSE The Clinic Manager is responsible for the safe and timely delivery of all patient care and technical procedures at the assigned OPKC facility. The related responsibilities include continuously staffing the facility with qualified, properly trained personnel; ensuring safety; maintaining proof of all necessary licenses and certifications for the employees and the facility; coordinating and scheduling patient care both within the facility and by referrals to other providers; maintaining employee and patient records; effectively communicating with patients and staff; and developing and implementing systems and procedures to ensure the safe, continuous quality improvement of all OPKC patients receiving dialysis services at assigned facility, effective and efficient operation of the facility. The Clinic Manager assumes these responsibilities twenty-four hours per day and responds immediately when needed, either directly and in person, or by delegation to other adequately trained and authorized supervisory staff. The Clinic Manager will supervise all OPKC staff at the assigned facility and works in collaboration with the OPKC Administrator, Director, Clinical Services, Medical Directors and other members of the OPKC management team. Supervisor: Director, Clinical Services QUALIFICATIONS Education: ? Degree from an accredited school of nursing. ? Bachelor’s degree in nursing preferred. Experience: ? Minimum two year’s work experience as a Registered Nurse. ? Minimum two year’s dialysis experience at an approved ESRD facility. ? Must have previous charge nurse experience. ? Previous nurse management experience preferred. License/Certification: ? Current RN license in the state of Washington. ? Current certification of Cardio Pulmonary Resuscitation (CPR) with a Basic Life Support (BLS) for Healthcare Providers card. ? National certification as CHN, CNN, CDN, or CPDN preferred. Responsible for developing and implementing policies, procedures, protocols and orders as approved by Administrator, COO and Medical Directors. ? Supervises, monitors and participates in all OPKC services and programs, including but not limited to: ? Outpatient dialysis ? Administration of medications ? New patient orientation ? Vascular access ? Anemia management ? Dialysis adequacy ? Iron stores ? Nutrition ? Medical records ? Laboratory testing, reporting and recording ? Emergency services and disaster planning ? Transportation ? Ensures the safety, effectiveness and availability of equipment and technical systems required by the facilities through periodic consultations with the Operations Coordinator and periodic review of equipment records and incident reports. ? Establishes and maintains a system that ensures and facilitates the transfer of patients into and out of the facility. ? Monitors and periodically reviews all patient charts to ensure accurate completion according to OPKC procedures and generally accepted practices. ? Makes rounds with physicians when requested. ? Establishes and maintains a good working relationship with all physicians on medical staff. Takes physician orders, instructions and calls, and intervenes when necessary to maintain effective working relationships between physicians and OPKC staff. ? Maintain strict adherence to infection control practices including handwashing, aseptic technique, and use of personal protective equipment (PPE). ? Recruits, interviews and selects employees for approved positions, involving other staff when appropriate. ? Responsible for ensuring employees are trained using OPKC approved materials, plans, schedules and tests required for employee orientation and training. Coordinates all training, using other staff as needed and participating directly. Ensures all training is documented. ? Responds to all staff requests for assistance, information and advice needed to ensure good patient care. ? Circulates through the dialysis unit daily, observing and interacting with dialysis staff. Spends such time as is necessary to be considered helpful and accessible to staff. ? Always treats staff with respect and courtesy. ? Assists in the maintain system to record and track all necessary employee data, including but not limited to: ? Current licenses and renewal dates ? Vaccinations and periodic medical tests ? CPR certifications and renewal dates ? Counseling and disciplinary actions ? Attendance record ? Performance evaluations ? Protects the confidentiality of all sensitive employee information, verbal and written. ? Ensures employees maintain accurate and timely documentation into patients’ Electronic Medical Record ? Completes and ensures the timely processing of Personnel Action Forms for all indicated changes, including changes in salary, employment status and selected insurance coverage. ? Develops the monthly staff schedule in concert with other OPKC Clinic Managers and Director, Clinical Services and posts in a timely manner. ? Accepts, reviews and processes PTO requests in a timely manner. Acknowledges receipt of all requests and conveys the status of each request to the employee within a week of receipt. ? Assists in development, implementation and participates as requested in a program of continuing staff education. ? Organizes, schedules and presents periodic staff meetings to obtain staff input and inform staff about changes and any active issues. ? Through direct observation and frequent contacts with the Charge Nurses, Clinical Coordinator, patients and physicians, ensures staff compliance with approved policies and procedures, safety standards and generally accepted good medical practices. Follows OPKC policies to teach, advise, counsel, warn and discipline employees as needed to ensure compliance. Documents actions in personnel files when indicated. Follows up on actions with additional observation and discussion. ? Conducts and completes annual performance-based employee evaluations on subordinates. ? Monitors staffing levels and patient census and makes staffing recommendations to the Director, Clinical Services. Strives to identify the need for additional positions before they are critically needed. ? Participates in the development, scheduling and staffing of on-call nurse coverage for the facilities' patients and for after-hours staff scheduling changes. OLYMPIC PENISULAR KIDNEY CENTERS JOB DESCRIPTION Other Functions: ? Knows and understands current Federal Part 405, 410, 413 et al, Conditions for Coverage for EndStage Renal Disease Facilities. Regularly monitors compliance. ? Knows and understands current CMS Interpretive Guidelines - End Stage Renal Disease Facilities. Initiates actions as needed to ensure compliance. ? Attends licensing and certification surveys and inspections. Attends related entrance and exit interviews and subsequent follow-up meetings. In collaboration with Director Clinical Services, Administrator and Medical Director, reviews and responds to any Statement of Deficiencies, and initiates, documents and reports all corrective actions and required monitoring. ? Ensures the timely completion and submittal of all ESRD Network reports and forms. ? Accepts reviews, maintains and responds to incident reports. ? Maintains and verifies physician information required for admitting privileges. ? Supervises the Quality Assessment Performance Improvement program at facility. Compiles, either directly or by delegation, statistics needed to monitor OPKC performance relative to the current CMS comparative data. Schedules and coordinates periodic meetings to present and review performance comparisons. Identifies and initiates actions to improve performance relative to established guidelines. ? Establishes and directs standing and temporary employee committees, as needed. ? Approves the biweekly time and ensures timely completion and submission for assigned personnel, tabulates, completes and submits the biweekly time utilizing Paycom. ? Participates in financial planning and budgeting. ? Tries to improve efficiency, and containment and reduction of operating costs. ? Regularly meets with and reports to Administrative staff. Attends Board of Directors meetings when requested. Advises and accepts direction from Administrative staff, Medical Directors and corporation Coordinators. ? Maintains a professional, even-handed, informed, authoritative and respectful demeanor at all times. Establishes and demonstrates the standard of behavior and performance expected from all employees. ? Participates in developing performance-based employee evaluation criteria. ? Identifies and completes, either directly or by delegation to others, such additional tasks needed for the day-to-day operation of the facilities. ? Accepts and completes such other tasks that are reasonably related to the operations of the dialysis facilities which may, from time to time, be assigned. Training: The Clinic Manager must complete all training described in the Dialysis Nurse job description, or an approved equivalent, within 90 days of accepting the Clinic Manager position. Employees who have successfully completed their orientation period and provided this advance notice of resignation, will receive the value of any accrued Personal Time-Off (PTO).
